Individual - An individual Albertan or couple, of any age, who provides volunteer service, demonstrates philanthropy, or exhibits outstanding service to seniors in Alberta.

Business - An Alberta business or corporation that has exhibited excellence in innovation or philanthropy in support of Alberta’s seniors.

Alice Modin - An individual Albertan or couple who is 65 years or older and has been volunteering in their community for 20 or more years; actively promotes volunteerism; and/or has had a provincial impact through their volunteer efforts.

Nonprofit Organization - An Alberta nonprofit organization that exhibits excellence in innovation or outstanding service to Alberta’s seniors.

Individual and Alice Modin - By selecting this option, you acknowledge that you are nominating this individual, or couple, for both an Individual Award and an Alice Modin Award. You also acknowledge that this nominee meets the criteria for both categories, which are as follows:

  • Individual: An individual Albertan or couple, of any age, who provides volunteer service, demonstrates philanthropy, or exhibits outstanding service to seniors in Alberta.
  • Alice Modin: An individual Albertan or couple who is 65 years or older and has been volunteering in their community for 20 or more years; actively promotes volunteerism; and/or has had a provincial impact through their volunteer efforts.

Tips for Applying:

Make sure your nomination package is complete.

The nomination:

  • Demonstrates how the nominee provides a service for the benefit of seniors;
  • Includes a summary;
  • Is legible in English, or has an English translation provided; and
  • Includes a completed Nomination Form that provides the name and address of the nominee, nominator, a checked consent box and is signed by the nominee, nominator.
  • Includes letters of support (optional).

The nomination summary should:

  • Describe why you are nominating this individual, business, or nonprofit organization;
  • Outline the volunteerism, philanthropy, innovation, or outstanding service in support of Alberta’s seniors;
  • Describe the type of work that was done or services provided, and the impact of that work; and
  • Provide any additional relevant information.

Upload documents:

  • You can upload documents in these file formats: PDF, JPEG, PNG, or TIFF.
  • Submit a maximum of 5 files per session.
  • All files submitted at the same time can total 120 MB.
  • Maximum file size is 120 MB.
  • Make sure your files are not password protected.
  • Once you've downloaded your completed nomination form, make note of the file number to upload more documents.

Alberta Seniors, Community and Social Services collects personal information about the Minister’s Seniors Service Award nominees and nominators pursuant to section 33(c) of the FOIP Act. The information collected will be used for the purposes of administering the Minister’s Seniors Service Awards program, determining the nominee’s suitability and eligibility to receive a Minister’s Seniors Service Award, promoting the program and its related events, and for other reasonably related administrative purposes in accordance with the FOIP Act.

I hereby freely consent to the disclosure, as the Ministry deems appropriate, of my name, address, phone number, photograph, quotation, and any information provided as part of this nomination package, to interested parties including MLAs and the media for interviews, stories and follow up. I hereby give permission to the Government of Alberta to use the aforementioned materials without any compensation to me. I understand this material will be used in communication and materials about Government of Alberta programs and services and they will be distributed to the public through a variety of means, including printed and electronic communications. All government communications where this material will appear shall constitute the property of the Government of Alberta, solely and completely. I understand that the material may be used by or licensed to other public bodies and private companies for use in materials in promoting the province of Alberta.

I waive all moral rights, claims, and objections arising from the use of this material, worldwide and in perpetuity, in favour of the Government of Alberta, its agents, employees, and contractors.
